You might want to start using an overnight mask for your hair. John Frieda makes one, it's sort of like a skin lotion, but for your hair. Using this should increase the moisture content of your hair, making it more resistant to breakage (I saw significant results within two weeks i.e. when I combed it, less hair came away)
In addition, reduce 'damaging behaviour' - straighten/curl your hair less, use a heat defense product/product with sunscreen to protect it from the elements e.g. sun and wind, sleep on a silk pillow case or wrap your hair in a silk scarf at night.
If you need to trim split ends, then do it. Otherwise it will slow down the process of recovery.
You don't have to do every single one of these, but a few routinely should result in a difference. I know it may seem like a lot but from the sound of it, your hair is damaged. So it will take more than a usual routine to get it back on track.
I hope that helps
